New Patient Hub

Hello and welcome to the new patient center. Thank you for choosing H2T PT for your chiropractic and physical therapy needs. In order to expedite your appointment and get you feeling better faster, we welcome you to fill out your new patient paperwork prior to your visit. If you are able to, please print it and have it with you along with a picture ID for your first visit. If not, there is no need to stress, we will be happy to provide you with the paperwork when you arrive.
